2nd Grade

1. Proof of well child physical within the past year.

7th Grade

1. Proof of 7th grade mandated immunizations.

2. Proof of well adolescent physical within the past year.

3. Proof of dental examination within the past year.

12th Grade

1. Proof of 12th grade mandated immunizations.

2. Proof of adolescent physical within the past year.
